Primary Services:
Independent Living Services will provide the
following services for people in order to accomplish our mission.

INTEGRATED SUPPORTS/SUPPORTED LIVING:
Services that provide support to allow the
individual to live in the most independent setting consistent with their abilities.
Integrated supports, include, but are not limited to services that the Developmental
Disabilities Services state agency describe as Medicaid Waiver Services. Supported Living
and Integrated Supports are provided in the following locations:
ILS owns and/or manages four group
homes and an eighteen unit apartment complex in Conway and an
eighteen unit apartment complex in Greenbrier. Along with many small
Waiver homes within the community. They are:

Ross House a group home for eight men;

Reynolds House a group home for eight women;

Patterson House a group home for 12 women;

Schneider House an intermediate care facility for the mentally retarded
(ICF/MR) for four men and six women that have serious medical needs in addition to the
developmental disability;

ILS Conway Apartment
Complex has 16 single and one two-bedroom apartments, a total of 17 apartment
units for residents.

Greenbrier Apartment Complex has
18 single unit apartments.
ILS provides services in the homes of people with
disabilities. Many people live independently in their own home/apartment in the
County. ILS staff assist people with managing their money, meeting health care,
nutritional, transportation, recreational, guidance and personal care needs. ILS
currently provides services to many families and/or individuals.
ILS provides services for people who need
24 hour supported living services in the community. ILS provides, trains and
supervises staff to serve people on an individual basis according to
their needs. This is provided as a Medicaid Waiver service option to congregate
living for people with severe disabilities. Currently only people who are
Medicaid eligible can receive this service option due to lack of funding.

AUXILIARY SERVICES:
People with disabilities who
receive services from ILS are assisted in their daily lives
several ways with additional auxiliary
services as needed to supplement living in the community.

PERSONAL CARE:
Personal Care Assistants, trained and supervised by ILS, assist
people with activities of daily living in their places of residence. This includes
assistance with grooming, bathing, laundry, housekeeping, meal preparation, feeding and
shopping, as well as monitoring health, medication
assistance, and the general well being of the individual. (Medicaid Generic)

CASE MANAGEMENT: Every person who receives services from ILS has case
management services. Case managers coordinate services within the
ILS agency. They also coordinate and connect people with services provided by others. Case managers may also assist people who are waiting for ILS services. Case management
includes a information and referral process to other agencies that are providing services
for the individual, as well as when a person is referred/transferred to another place of
residence. ILS will
perform all case management functions described as necessary in DDS/Medicaid Policy and
Standards in order to assist the individual/family in having a good/better quality of

ADULT DEVELOPMENT: Profiles
is ILS's Adult Development Program. Profiles provides services to adults,
age 21 or older or who are holding a certificate of completion/graduation from a public
school and who have been diagnosed with a developmental disability.
These services will be provided with a curriculum that has a wide variety of topics
dealing with living independently in a community, getting a job and having meaningful
relationships with people. Learning will be accentuated by using video equipment, computers and other
technologies available for teaching/training people with developmental disabilities.

DIAGNOSIS AND EVALUATION:
This process includes a complete analysis of the
individuals aptitude and interests, identifying special skills, what he/she hopes for and
needs, identified through an interview process using a functional analysis of living
skills as a guide. This information will be formulated into a habilitation plan for the
individual with the help of those people who he/she selects to assist him/her. The process
will include other information such as psychological tests, medical diagnosis/history,
and/or any other information that would assist in establishing a reasonable plan for
progress in the life of the person. The goal of the D & E is to provide information
for use in establishing a training/assistance regimen that will empower the person help
themselves in becoming more independent. D&E/AD 10/24/96

TRANSPORTATION:
ILS provides transportation to all who receive services. Lack of
adequate staff and vehicles limit transportation services provided to the City of Conway.
The first priorities to receive transportation services are for food, health, clothing and
employment needs for the individual. Other activities identified on the service plan, i.e.
recreational and leisure activities, etc., are worked into the schedule as time allows.

PROCEDURES:
In order to access residential services provided by Independent
Living Services, Inc. there are several procedures. They are explained in the Application
material, provided on request. They are: Application/In Take; Waiting List; Admissions
Process (Service Requirements, Releases, House Rules, etc.); and Move In/Intake. After the
Move In, an assessment of daily living skills is completed. Then services are identified
and coordinated according to what people want and need.

SERVICE
LIMITATIONS:
ILS reserves the right to deny housing
services to anyone who doesn't meet the eligibility criteria of developmental disability;
anyone who has service needs beyond what ILS service personnel can safely/adequately
provide; anyone refusing to recognize and follow ILS rules and laws of the community;
anyone who has a history of being dangerous to themselves or others.

FEES
FOR SERVICES: Rent (30% of gross income) is collected for
all HUD facilities; an additional fee for food & services for people who live in
Ross, Reynolds & Patterson group homes. Only people who are "low Income" are
eligible for HUD subsidized units. Housing & Urban Development (HUD) only allows
people who are 18 years or age and older to rent its residential units. A parent and child
can live in the two bedroom unit if approved by HUD.
